During the first visit, I will take the time to learn more about you and your symptoms. I will obtain important background information, like your medical history, and give you time to get to know me as you new primary care doctor.
What to expect
Being prepared for your appointment will ensure that I have all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Save some time at your first visit by printing out and completing the new patient forms in advance of your appointment. I look forward to your first visit.

Please bring the following with you to your appointment
- - Your insurance card
- - A valid, government-issued photo-id card
- - Any prescription medications you are currently taking
- - A copy of your medical records from your previous doctor
- - All co-pays are due in cash or credit card only before seeing the doctor
